To understand the clinical manifestations, differential diagnosis, and treatment of seborrheic dermatitis. Clinical Case A 22-year-old male presents with a rash on his face and chest. Physical exam shows dandruff on his scalp and a scaly rash on erythematous skin around his eyebrows, nasolabial folds, forehead, and behind his ears. He has a similar rash on his sternal area. 1. What are the differential diagnoses you would entertain? Which is the most likely diagnosis? 2. What are the treatment options and how would you approach treating the various affected sites? 3. If the above patient had an acute and explosive onset of the symptoms what other associated diagnosis would you consider? 4. What other illnesses are commonly associated with this skin problem? 5. When do you refer to a dermatologist? Teaching Points Seborrheic dermatitis is a chronic inflammatory disorder affecting the scalp, face, and trunk and other areas where sebaceous glands are most prominent. Scalp seborrhea presents as mild dandruff to dense diffuse adherent scales. In infants this is called cradle cap. Facial and truncal seborrhea presents as greasy scales on erythematous inflamed skin along the skin folds and hair margins. Etiology The etiology is not well understood.
