This article suggests Vit D supplementation to all infants since 2 months of age, especially to those who is exclusively BF. They also note, that breast milk has insufficient level of Vit D to prevent rickets and there is no way to increase its level in breastmilk. There is a statement that dermatologists advise do not expose the child below 6 months to the sun, therefore there is no alternative, but to supplement a child with Vitamin D. I see that the AAP is concerned about the rise of the rickets, but the information which is given in the article is too single minded and incorrect( or I might be very outdated in a literature on breast milk). I remember we had similar discussion with friends from WHO and some nutritionists and what came in mind as a solution for "rickets enigma even in sun abundant countries" was to continue BF as it has soluble forms of the Vitamin D and to expose child to the sun( Ok after 6 months of life if a baby is very fair, but I never heard about this restriction) I vaguely remember an article which was cited very often that suggested, that to built sufficient amount of vitamin D, it is enough to expose the child's hands and face only to the sun as little as 10 minutes a day or 30 minutes a week even ( or 2 hours a week)even in the Northern altitudes( black babies probably would need more), it seems 10 minutes a day is no risk for a skin cancer. Hope I am right, rickets is a very bad thing to have and it should be prevented by all means, but mother should be given a full and correct information. I also think that pills are too simple alternative and a compliance is an issue and it is too medicalized approach, while if we give some alternative suggestions how to build up Vitamin D level continuing breastfeeding and expose the child to the sun, then there is better chance to succeed.
