Thrombosis Risk Factor Assessment / Guidelines Choose All That Apply Each Risk Factor Represents 1 Point Each Each Risk Factor Represents 2 Points Each Minor Surgery Planned Patient is 61-74 years old Patient is 41-60 years old Malignancy-except skin (present or previous) History of prior major surgery (<1 month) Major Surgery (>45 minutes) Varicose Veins Laparoscopic surgery (>45 minutes) Inflammatory Bowel Disease Patient confined to bed (>72 hours) pre & post admission Swollen legs (current) Lower extremity immobilizing cast (<1 month) Obesity Central venous access (<1 month) Oral contraceptives or hormone replacement therapy Each Risk Factor Represents 3 Points Each Each Risk Factor Represents 5 Points Each History of DVT/PE Elective total hip or knee replacement Family history of thrombosis Total joint replacement or hip, pelvis, or leg fracture (<1 month) Patient is 75 years old or greater Stroke (< 1 month) Major surgery with additional risk factors: MI, CHF, Sepsis, or serious lung disease (abnormal pulmonary function-COPD) Multiple trauma (<1 month) Medical patient with additional risk factors: MI, CHF, history of Stroke, Sepsis, serious lung disease or pneumonia PATIENT WEIGHT: History of increased clotting Total Points: Nurse Signature: _______ CONTRAINDICATIONS FOR MEDICATION PROPHYLAXIS Coumadin®: Pregnancy, Blood Dyscrasias, Bleeding tendencies-active ulceration, Spinal puncture, Unsupervised senile patient and recent /Contemplated major surgery Lovenox®: Active major bleeding, Thrombocytopenia associated with positive antibody test (HIT), Known hypersensitivity to heparin or (porcine) pork products OR Total Points Risk Level Prophylaxis Regimen Guidelines 0-1 Low Risk No specific measures; ...
